Adult Worker Forms

All workers must complete (1) the Adult Worker Profile, (2) the Form 79 either for Masons or Non-Masons, as applicable, and (3) the Code of Conduct.

All workers must complete an online background check.  The results will be delivered directly to the Supreme Inspector.  Click the button below to begin the process. You will create an account, enter the requested information, pay and submit. - very easy!
Background Check

All workers must subscribe to the Texas Rainbow News for the year for which they will serve.  Please submit your subscription to Ms. Suzette Faith prior to submitting your Adult Worker Profile, Form 79 and Code of Conduct to the Supreme Inspector.
